The crew at Valve have been working diligently to fix an issue that recently popped up with many users' Steam friends. Mike B. from Valve issued (http://forums.steampowered.com/forums/showpost.php?p=13665636&postcount=2316) this following statement that explains what will be going on for the two stage fix.
The fix is coming in two parts. The first part was to post code this morning during the downtime that enabled the reset of stuck relationships, and also helped with the timespan of the ignoring relationship. The ignoring duration isn't a substantial issue, but I think it was affecting a few users. The code to free stuck relationships enables the second half of the work.
The second half of the work is to traverse the database looking for users who have invalid relationships and reset them. I'm running code that will do this for everyone, and work as it goes. I can post later about how long it will take after measuring its progress for a while, and perhaps tuning its performance a bit.
Point is, you'll start to see the relationships automatically repaired over time going forward.
Some users may have to accept a few outstanding invites for friends, even if they were already on their friends' list prior to the glitch. If you haven't already received your fix, it should be coming to you shortly.
