Mods normally have instructions that tell you how to install them. Djuice's Realism Mod has a readme.txt. If they don't use an installer .exe file they just unzip to a folder in the mods folder; usually you need to run a setup batch file to copy vanilla files into the new folder. (BTW, Djuice stopped developing his mod and joined the Project Reality mod team.)
